De jonge, idealistische verpleger Ollie Cross (Tye Sheridan) loopt mee met de cynische, doorgewinterde ambulanceveteraan Gene Rutkovsky (Sean Penn). Hij wordt geconfronteerd met de genadeloos harde realiteit van het werk van het ambulancepersoneel en de tol die deze zware baan mentaal en fysiek van hen vergt. Oog in oog met het drugsgebruik en de criminaliteit die straten van New York eigen zijn, begint Ollie's perspectief op leven en dood langzaam te veranderen...
